進出口貨運險基本索賠材料清單

集裝箱整箱運輸，如貨物發(fā)生碰損、濕損等，通常需提供以下資料:

1.正本保單一聯(lián)；original policy of insurance

2.正本提單一聯(lián)（如是電放，須提供清晰的帶有電放章的復(fù)印件）；original bill of lading (If B/L

surrendered/telex-released, please give us a clear copy of B/L with the stamp of

surrendered/telex-released)

3.正本發(fā)票和正本裝箱單；Original commercial invoice & packing list with shipper’s stamp

and signature

4.買賣合同復(fù)印件（需在復(fù)印件上蓋章，如合同是多頁，請加蓋騎縫章，并注明“與原件相符”）；copy

of sales contract with seller’s original signature and stamp on the copy and with the

statement “match with the original one”

5.出口報關(guān)單復(fù)印件；copy of export customs declaration form

6.詳細說明貨物包裝情況，如包裝方式、材料等；以及貨物的裝箱情況，如貨物在何時何地由誰裝箱，

并詳細說明貨物在集裝箱內(nèi)的積載、綁扎、固定情況（由發(fā)貨人簽字、蓋章）；shipper’s statement

about the package pattern of the cargo, packing material, the stowage inside the container,

security fixation and where and when the container was stuffed by whom(with the shipper’s

original signature and stamp) ；如有裝箱照片或監(jiān)裝報告請一并提供；if any loading report or

photos before shipment please also submit

7.集裝箱裝箱單；container load plan

8.裝運港場站收據(jù)；dock receipt at loading port；或裝運港的進出堆場的設(shè)備交接單EIR(向負責運

送至堆場的司機要) EIR(equipment interchange receipt) from and back to CY/port at the

loading port

9.卸貨港的進出堆場的設(shè)備交接單EIR(向去卸貨港拉貨的司機要) delivery receipt or

EIR(equipment interchange receipt) from and back to CY/port, which could be got from the

driver who picked up the container from CY/port

10.收貨人收貨時與卡車司機之間的貨物交接單；delivery receipt/note between the consignee

and the inland driver

11.收貨人說明集裝箱到達收貨人處時外表及鉛封狀況（由收貨人簽字、蓋章）consignee’s

statement about the condition of the container when arrived(with consignee’s signature or

stamp)

12.開箱時集裝箱照片、貨損照片；color photos of the damaged cargo and the container when

opened the container

13.收貨人與承運人（和其他責任方）關(guān)于損失索賠的函件、傳真等；correspondence exchange

with the Carriers and other Parties regarding their liability for the damage

14.收貨人索賠函和索賠清單（須詳細列名索賠事項和金額，并由收貨人簽字蓋章）；consignee’s

detailed claim letter to Ping An which clarify the damage situation and the claim amount,

with consignee’s signature and/or stamp

15.如發(fā)生殘值處理或維修，請?zhí)峁┫鄳?yīng)的發(fā)票； if salvage action or recondition, please submit

the relevant invoices

海運集裝箱整箱運輸，如貨物發(fā)生短少，通常需提供以下資料：

1.正本保單一聯(lián)；original policy of insurance

9.航空公司或其代理出具的貨損/貨差證明，或收貨人已收到航空公司已賠付貨損的賠款收據(jù),或航空公

司對收貨人索賠做出賠付承諾的書面回函；the damage/loss report from the airline company or

its agent, or the payment receipt signed by the consignee which means that the carrier has

paid the damage/loss according to the clause of the waybill, or the airline company’s

written confirmation of paying to the damage/loss to the consignee

10.收貨人與承運人（和其他責任方）關(guān)于損失索賠的函件、傳真等；correspondence exchange

with the Carriers and other Parties regarding their liability for the damage

冷藏集裝箱運輸貨物發(fā)現(xiàn)變質(zhì)、腐爛等，通常需要提供下列資料：
